Notesakes Order Form

Please fill out a separate order form for each order you are placing. Shipping costs vary depending on the quantity and sizes of items purchased. We will be happy to provide you an estimated shipping cost if you contact us at 601-826-3745 or via e-mail at info@notesakes.com. If you live in the local area, we can arrange for you to pick up the item and save on shipping costs. Once your form has been submitted, we will send you a final invoice. If you would like a pdf proof sent with your invoice, please indicate that below. Payment is expected before we print or deliver your product. You can mail your payment to Notesakes, P.O. Box 5905, Brandon, MS 39047-5905. We accept cash, checks, money orders and cashier's checks. Thank you!
